A Clinical Observation on Comatose Children


Abstract
The significance of coma, particularly in the pediatric age range, remains enigmatic. In contrast to adults, children experiencing coma have generally been thought to have a better outcome, but the outcome is more difficult to predict.
Present study was performed to identify outcome and the relevance of coma duration and causes as a prognostic factor.
A clinical analysis of 121 children who were comatose in admission to the Korea University Hospital during the period of January 1979 to December 1982 was made.
The results obtained were as fallows:
1. There were 71 males and 50 females and sex- ratio was 1.4 : 1. The highest incidence of comatose children was 6 months to 3 years of age group (40 cases, 33.1%) and 5 years to 10 years of age group(37 cases, 30.6%) was the next. Seasonal peak was in autumn.
2. Comatose state was mainly caused by convulsive disorder(27 cases, 22.3%), meningitis (21 cases, 17.4%) and Reye syndrome(lo cases, 8.3%).
3. According to analysis of consciousness on admission, 46 cases (38. 0%) were drowsy state, 26 cases (21. 5%) confused state, 22 cases (18. 2%) stuporous state, 17 cases 14. 0%) semi coma state and 10 cases(8.3%) were come state.
4. Complete recovery was found 89 out of 121 cases (73.5%), 22 cases (18.2%) were discharged against advice and 10 cases(8.3%) were not recovered.
5. 50 cases were recovered within 24 hours, 29 cases were 1 to 3 days, 5 cases 3 to 5 days and 5 cases were after 5 days. The best level of recovery was related mainly to cause and duration of coma.
6. The overall mortality rate was 8.3% and the highest mortality observed in cases of Reye syndrome and liver disease.
